The Chicago Blackhawks and BMO Harris Bank are proud to honor members of the hockey community. Each month during the regular season we recognize the excellence of a youth hockey player, coach and volunteer, in addition to a Blackhawks player! 

BMO Harris Bank Youth Player/Coach/Volunteer of the Month

Nominate your favorite youth hockey player, coach or volunteer and they could win tickets to a Blackhawks game plus other great prizes!

Criteria is based on hockey accomplishments, volunteer activities in the nominee's community, demonstrated passion, dedication and spirit through commitment to community. In determining the Youth Hockey Player of the Month, the judges will also consider the nominee's academic accomplishments and involvement with his/her school.

Official Rules

1. How to Enter: No Purchase or Transaction Necessary to Enter or Win. Purchases or transactions will not increase your chances of winning. From August 19, 2019 through March 6, 2020 ("Contest Period"), fans can submit nominations for the BMO Harris Youth Hockey Player, Coach and Volunteer of the Month ("Contest"). The nomination form is available on-line at chicagoblackhawks.com (located in the youth hockey and contest sections), and in various Chicago-area ice rinks. During the Contest Period, fans can (1) submit a nomination form on-line at chicagoblackhawks.com by following the online instructions, or (2) complete a nomination form or hand-print or type on a 8 ½ by 11 inch sheet of paper the name of the nominator, the name and age (if nominee is for youth hockey player of the month) of the nominee, the nominee's youth hockey team and division, the nominee's hometown, category of the nomination (player, coach, or volunteer), the nominator's relationship to the nominee (including the length of time the nominator has known the nominee), and the reason for the nomination, and mail it to the Chicago Blackhawks at 1901 W. Madison Street, Chicago, IL 60612. All entries must be received by the last day of the Contest Period. No mechanically reproduced or photocopied entries permitted. Entries become the property of Chicago Blackhawk Hockey Team, Inc. ("Chicago Blackhawks") and will not be returned. This contest is sponsored by Chicago Blackhawks and BMO Harris Bank N.A. (collectively "Sponsors").

2. Eligibility: Nominees for Youth Hockey Player of the Month must be at least five (5) years of age and younger than eighteen (18) years of age as of the date of his or her nomination. Nominees for Youth Hockey Coach of the Month must be eighteen (18) years of age or older as of the date of his or her nomination. Nominees for Youth Hockey Volunteer of the Month may be any age. Nominees must be individuals and they and their immediate families (defined as spouses and individuals living in the same household) must not be employees, directors, officers, governors or agents of (a) either Sponsor, (b) the National Hockey League ("NHL"), (c) the NHL's member teams, (d) Sponsors' advertising and promotion agencies, or (e) any of their respective parents, subsidiaries, or affiliates. Nominators must be individuals (i.e. not a company, foundation or other organization), over 18 years of age, and they and their immediate families (defined as spouses and individuals living in the same household) must not be employees of (a) either Sponsor, (b) the NHL, (c) the NHL's member teams, (d) Sponsors' advertising and promotion agencies, or (e) any of their respective parents, subsidiaries, affiliates, directors, officers, governors and agents. Both nominees and nominators must be legal residents of Illinois, Indiana or Wisconsin. All entries become the property of the Chicago Blackhawks.

3. Judging Criteria: During the Contest Period, judges selected by the Chicago Blackhawks will select six (6) Youth Hockey Players of the Month, six (6) Youth Coaches of the Month, and six (6) Youth Hockey Volunteers of the Month (each, a "Contest Winner") in their sole discretion. Contest Winners will be selected based on the following criteria: (a) hockey accomplishments; (b) volunteer activities in the nominee's community; and (c) demonstrated passion, dedication and spirit through commitment to community. In determining the Youth Hockey Player of the Month, the judges will also consider the nominee's academic accomplishments and involvement with his or her school. The judges will select three (3) Contest Winners (one player, one coach and one volunteer) on six (6) dates (the "Selection Date(s)") from among all eligible entries received prior to such Selection Dates. The Selection Dates shall be determined by the Chicago Blackhawks in its sole discretion.

Non-winning entries submitted prior to a Selection Date will be carried over and considered eligible to be selected as a winner on the next Selection Date, if any.

The odds of winning will depend on the number of entries received.

4. Prize: The Contest Winner will receive the following: (a) the Contest Winners will be honored on-ice prior to a Chicago Blackhawks game, and the presentation will incorporate the film of each Contest Winner, (b) four (4) 100 level game tickets to the Chicago Blackhawks game at which he or she will be honored (seat location to be determined by Chicago Blackhawks in its sole discretion), (c) a $100 BMO Harris Bank gift card, and (d) a plaque (collectively, the "Prize"). The dates of the award presentation games will be determined by the Chicago Blackhawks in its sole discretion. Presentation dates are subject to change in the Sponsors' sole discretion.

As part of the Prize, the Chicago Blackhawks will send a certificate, letter and photo to each of the eighteen (18) Contest Winners' hockey organization(s) congratulating them. Additionally, the Contest Winners will have the opportunity to attend a Chicago Blackhawks practice (one session for all Contest Winners). The date of the practice shall be determined by the Chicago Blackhawks in its sole discretion. Finally, the Chicago Blackhawks will post a head shot, biography and short video clip about the each Contest Winner on the youth hockey section of the Chicago Blackhawks' website. The approximate retail value of each Prize will vary depending on which game the Contest Winner attends, and ranges between $901.92 and $1,022.88.

Each nominee is only eligible to win one Prize.

All incidental costs and expenses not specifically referred to as being included in the Prize, including, but not limited to, transportation to and from the United Center, parking, food, beverages and souvenir items, and all taxes associated with participation in or receipt or use of a prize from the contest shall be the responsibility of the Contest Winner. An IRS Miscellaneous Income Form 1099 will be issued to each Contest Winner. Winners are advised to seek independent counsel regarding the tax implications of accepting the Prize. Sponsors, in their sole discretion, may substitute or alter the Prize for one of equal or greater value. The Prize is non-transferable and cannot be sold or exchanged for cash or otherwise, except at the sole discretion of Sponsors.

5. Awarding of Prize: Selected Contest Winners will be contacted by telephone or e-mail based on the contact information provided in the entry form. The Contest Winner's tickets will be available for pick-up at the United Center at Will Call prior to the game for which the tickets are provided. Valid state or federal identification from the Contest Winner, or one of the Contest Winner's parents or legal guardian, is required to pick up the tickets at Will Call. Contest Winners must attend the game for which game tickets are provided.

6. General Rules: By entering this Contest, the nominator agrees (and by accepting the Prize, the Contest Winner agrees) to be bound by these Official Rules and by the decisions of the Sponsors, whose decisions are final with respect to all matters relating to the Contest, including without limitation, eligibility and /or disqualification of entries. Entries are subject to verification and are automatically void if forged, altered, or irregular in any way. Sponsors, in their sole discretion, may disqualify any nominator and his or her nominee who is found to have tampered in any way with the operation of the Contest.

The Sponsors are not responsible for (i) lost, late, damaged, destroyed, inaccurate, delayed, incomplete, unintelligible, non-delivered, misdirected or stolen entries; (ii) interrupted or unavailable network, broadband, satellite or telephone networks or lines, computer systems, computer equipment, software, viruses or bugs, servers or providers, or other connections, availability or accessibility; (iii) unauthorized access to, or alteration of, entries; (iv) miscommunications, failed computer, telephone, cellular, satellite, or cable transmissions, lines or other technical failure; (v) jumbled, scrambled, delayed, or misdirected transmissions, computer hardware or software malfunctions, failures or difficulties; (vi) any other errors of any kind, whether human, technical, mechanical, electronic or network, including, without limitation, any errors that may occur in connection with the administration of the Contest or in any Contest-related materials; or (vii) the incorrect or inaccurate capture of entry or other information, or the failure to capture any such information.

Each Contest Winner (or the Contest Winner's parent or legal guardian if winner is a minor) must complete, execute and return an Affidavit of Eligibility and Publicity/Liability Release (collectively, the "Release"), without revision, within seven (7) days of notification or the Prize will be forfeited and may be awarded to an alternative winner. Each Contest Winner must be able to attend the award presentation game at which Contest Winner will be acknowledged. In the event Contest Winner is unable to attend such award presentation game, Contest Winner will be disqualified and the Prize may be awarded to an alternative winner. By accepting the Prize and executing the Release, the Contest Winner, among other things: (a) agrees to hold harmless and release Sponsors, the NHL, the United Center Joint Venture, the Blackhawk Community Ice Rink, LLC, and each of their respective parents, subsidiaries, affiliates, directors, officers, attorneys, partners, equity holders, shareholders, members, employees, agents and representatives and all others associated with the manufacture, distribution, development and execution of this Contest and prizes (collectively the "Released Parties") from and against any and all claims, liability, injury, expenses (including reasonable attorneys' fees), damage, loss or causes of action (however named or described) that may occur, directly or indirectly, in whole or in part, from: (i) Contest Winner's participation in the Contest; (ii) the delivery, misdelivery, receipt, acceptance, possession, use or misuse of the Prize awarded in the Contest; (iii) the use of a Contest Winner's name, photograph and/or likeness as permitted herein, including, without limitation, claims based on publicity rights, defamation and/or invasion of privacy; and/or (iv) any travel or activity related to the receipt, use or misuse of the Prize, including, but not limited to, personal injury, death and/or property damage. Additionally, the Released Parties are not responsible for electronic communications that are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in entrant's e-mail account to receive e-mail messages. Released Parties are not responsible or liable to any nominee or winner or any person claiming through such nominee or winner for failure to supply the Prize or any part thereof or inability to continue the Contest, by reason of any act of God, any action, regulation, order or request by any governmental or quasi-governmental entity (whether or not the action, regulations, order or request proves to be invalid), equipment failure, threatened terrorist acts, terrorist acts, kidnapping, air raid, blackout, act of public enemy, earthquake, war (declared or undeclared), fire, flood, epidemic, explosion, unusually severe weather, hurricane, embargo, labor dispute, lockout, work slowdown or strike (whether legal or illegal), labor or material shortage, transportation interruption of any kind, civil disturbance, insurrection, riot, vandalism, or any other cause beyond the Released Parties' sole control. Those who do not follow all of the instructions, provide the required information in their entry form, or abide by these Official Rules or other instructions of the Chicago Blackhawks may be disqualified. The Chicago Blackhawks reserve the right in its sole discretion to disqualify any individual feared to be tampering with the entry process or the operation of the Contest or to be found acting in violation of the Official Rules, or to undermine the legitimate operation of the Contest.

In consideration for being awarded the Prize, each Contest Winner (or the Contest Winner's parent or legal guardian if Contest Winner is a minor) hereby agrees and consents, without further authorization, compensation or remuneration of any kind, to the use of the winner's name, photograph, biographical information and/or likeness in any and all advertising, promotions and other publicity conducted by Sponsors, the NHL and its member teams, NHL Enterprises, and each of their respective parents, subsidiaries, affiliates and designees. Each Contest Winner (or the Contest Winner's parent or legal guardian if Contest Winner is a minor) further acknowledges and agrees that such Contest Winner may be identified on the in-arena videoboard and announced as a BMO Harris "Youth Hockey Player of the Month," a BMO Harris "Youth Hockey Coach of the Month" or a BMO Harris "Youth Hockey Volunteer of the Month" on the public address system at the United Center at the game he or she attends as a prize winner.

This Contest is subject to all applicable federal, state and local laws and regulations. In the event Sponsors are prevented from continuing or completing this Contest, or the integrity and/or feasibility of the Contest is undermined by any event or cause not reasonably within the Sponsors' control, including but not limited to electronic disruption, virus, bugs, tampering, printing or production errors, work stoppage, lockout or other labor dispute, or other causes beyond Sponsors' control that disrupt the administration, security or proper play of this Contest, Sponsors shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel or terminate the Contest without further obligation. If the Sponsors, in their discretion, elect to abbreviate the Contest, Sponsors reserve the right, but not the obligation, to award the Prize from among all valid and eligible entries received up to the time of such event or cause. Sponsors are not responsible if any game is delayed or cancelled for reasons beyond their control.

All personal information collected from the Contest shall be subject to the terms of Chicago Blackhawks' Privacy Policy, which may be viewed at chicagoblackhawks.com.

7. Winners' List: To receive a full list of Contest Winners, send a postage-paid envelope to: BMO Harris/Blackhawks Youth Hockey Player, Coach and Volunteer of the Month Contest, Chicago Blackhawks, 1901 W. Madison Street, Chicago, IL 60612. Requests for a Winners' List must be received after April 1, 2020 and before June 30, 2020.

This Contest is administered solely by Chicago Blackhawk Hockey Team, Inc. BMO Harris Bank N.A. has no involvement in the administration of this Contest. All inquiries regarding the Contest should be directed to the Chicago Blackhawks at the address listed above.

OFFICIAL RULES

(1) No purchase necessary to enter or win. A purchase will not increase a person's odds of winning with an entry.  ODDS OF WINNING WILL DEPEND ON THE TOTAL NUMBER OF ENTRIES RECEIVED. ENTRY INTO THE SWEEPSTAKES CONSTITUTES ACCEPTANCE OF THESE OFFICIAL RULES. VOID WHERE PROHIBITED BY LAW.  

(2) The Blackhawks Player of the Month Sweepstakes (the "Sweepstakes") is open only to legal residents of the states of Illinois, Wisconsin or Indiana who are 18 years of age or older on the date of entry; provided that the following persons are not eligible to participate in the Sweepstakes: (a) the employees, officers, directors, partners, equity holders, shareholders and members of Chicago Blackhawk Hockey Team, Inc. ("Chicago Blackhawks") and BMO Harris Bank N.A. ("BMO Harris"), and their respective immediate family members (spouse, parents, legal guardians, in-laws, grandparents, siblings, children and grandchildren) and members of their same households (whether legally related or not), and (b) the employees, officers, directors, partners, equity holders, shareholders, and members of the National Hockey League (the "NHL") and its member teams, NHL Enterprises, L.P., NHL Enterprises Canada, L.P., NHL Enterprises B.V., and/or NHL Interactive CyberEnterprises, LLC (the foregoing are collectively the "NHL Entities"), and their respective immediate family members (spouse, parents, legal guardians, in-laws, grandparents, siblings, children and grandchildren) and members of their same households (whether legally related or not). The decisions of the Chicago Blackhawks, including, but not limited to, eligibility of participants and selection of the winners, are final and binding on all matters relating to the Sweepstakes. By participating in the Sweepstakes, each entrant unconditionally accepts and agrees to comply with and abide by these "Official Rules." 

(3) Sweepstakes period: The Sweepstakes begins at 12:00 a.m. Central Time ("CT") on October 14, 2019 and ends at 11:59 p.m. CT on April 1, 2020 ("Sweepstakes Period"). Chicago Blackhawks' computer is the official timekeeper of the Sweepstakes. 

(4) HOW TO ENTER: Individuals can enter online by visiting the Blackhawks webpage, http://www.chicagoblackhawks.com, participating in a poll to select the best Chicago Blackhawk player for that month, and completing an online entry form. Voting in the poll allows the individual to view the results of the poll. A vote in the poll and a completed entry form results in an entry, regardless of the entrant's choice for best Chicago Blackhawks player or whether the entrant's choice receives the most votes. In lieu of participating in the poll, an individual can enter the Sweepstakes by following the instructions on this webpage and completing an online entry form. The player with the most votes shall be the Blackhawks Player of the Month for that month. Once submitted, all entries become the exclusive property of the Chicago Blackhawks, and will not be acknowledged or returned. Proof of sending or submission will not be deemed to be proof of receipt by the Chicago Blackhawks. The Chicago Blackhawks are not responsible for entries that are incomplete, illegible, corrupted, damaged, destroyed, forged, false, lost, late or misdirected, deceptive or otherwise not in compliance with the Official Rules, and such entries will be discarded. No responsibility is assumed for technical, electronic, telephone, hardware, software, network computer or data transmission errors, malfunctions or difficulties of any kind. There is a limit of one entry per person per month. 

(5) WINNERS: The Chicago Blackhawks will randomly select a total of six (6) winners, one each for the months of October, November, December, January, February and March. On each of six (6) dates (the "Selection Date(s)"), the Chicago Blackhawks will randomly select one (1) prize winner from among all eligible entries received prior to such Selection Dates. The Selection Dates shall be determined by the Chicago Blackhawks in its sole discretion. Entries not selected on a Selection Date shall carry over to the next Selection Date, if any. Odds of winning a prize are dependent on the number of eligible entries received. Purchase will not increase odds of winning. For the name of the Sweepstakes winners, send a self-addressed, stamped envelope to the Chicago Blackhawks, Blackhawks Player of the Month Sweepstakes, United Center, 1901 West Madison St., Chicago, IL 60612 after December 1, 2019 and before April 30, 2020.  

(6) PRIZES: Each prize winner will receive a hockey puck autographed by the Blackhawks Player of the Month corresponding to the month for which the entrant won. The approximate retail value ("ARV") of each prize is $50.00.

(7) All prizes are non-transferable and non-assignable by winner to any other person, including relatives or friends, without the consent of the Chicago Blackhawks. All prize details not specified in these Official Rules will be determined by the Chicago Blackhawks in its sole discretion. Prize details and availability are subject to change. In the event a prize is unavailable, the Chicago Blackhawks reserve the right to substitute a prize of comparable or greater value. Unless otherwise stated herein, all prizes are awarded "AS IS" and without warranty of any kind, express or implied (including, without limitation, any implied warranty of merchantability or fitness for a particular purpose). All federal, state and local taxes are solely the winner's responsibility. Unclaimed prizes will be forfeited. One prize per person. The prizes are not redeemable for cash. 

(8) By entering, each entrant agrees to follow these Official Rules and to release, hold harmless and indemnify the Chicago Blackhawk Hockey Team, Inc., BMO Harris Bank, N.A., the NHL Entities, United Center Joint Venture, Blackhawk Community Ice Rink, LLC, and each of their respective parents, subsidiaries, affiliates, directors, officers, attorneys, partners, equity holders, shareholders, members, employees, agents and representatives and all others associated with the manufacture, distribution, development and execution of this Sweepstakes and prizes (the "Released Parties") from and against any and all claims, liability, injury, expenses (including reasonable attorneys' fees), damage, loss or causes of action (however named or described) that may occur, directly or indirectly, in whole or in part, from: (i) entrant's participation in the Sweepstakes;  (ii) the delivery, misdelivery, receipt, acceptance, possession, use or misuse of the prizes awarded in the Sweepstakes; (iii) the use of a winner's name, photograph and/or likeness as permitted herein, including, without limitation, claims based on publicity rights, defamation and/or invasion of privacy; (iv) entrant's actual or alleged misappropriation or infringement of proprietary, protected, trademarked, copyrighted or other intellectual property; (v) any travel or activity related to the receipt, use or misuse of the prizes, including, but not limited to, personal injury, death and/or property damage; and/or (vi) any other claims that relate in any way to the Sweepstakes. In the event of a dispute as to the identity of an entrant, entry will be declared made by the authorized account holder of the email account submitted at the time of entry. "Authorized account holder" is defined as the natural person who is assigned to an email address by a telephone company, wireless service provider, Internet access provider, Internet service provider or other organization (e.g. business, educational institution, etc.) that is responsible for assigning email addresses. Potential winner may be requested to provide Chicago Blackhawks with proof that he/she is the authorized account holder of the email account associated with the winning entry.

(9) In consideration for being awarded a prize, each winner hereby agrees and consents, without further authorization, compensation or remuneration of any kind, to the use of the winner's name, photograph, biographical information and/or likeness in any and all advertising, promotions and other publicity conducted by the Chicago Blackhawks, BMO Harris, the NHL Entities, and/or each of their respective parents, subsidiaries, affiliates and designees. Winner assumes all liability and risk for any injury or damage caused, or claimed to be caused, by participation in this Sweepstakes or the receipt, use or misuse of any prize. Potential winner will be required to sign and return an affidavit of eligibility, release of liability and publicity license ("Affidavit") form within a specified time after attempted notification. If any potential winner fails or refuses to sign and return such Affidavit within the time period required by the Chicago Blackhawks or if the prize or prize notification is returned as rejected, faulty or unclaimed, or returned as undeliverable to such potential winner, such winner may be disqualified and an alternate winner may be selected. If winner is found to be ineligible, or if he or she has not complied with these Official Rules, or if a winner declines a prize for any reason prior to the award, such winner may be disqualified and an alternate winner may be selected. Identity of the winner is subject to the Chicago Blackhawks' verification. The Chicago Blackhawks are not responsible for and shall not be liable for late, lost, damaged, intercepted, misdirected, or unsuccessful efforts to notify the winners. 

(10) All personal information collected from the Sweepstakes shall be subject to the terms of the Chicago Blackhawks Privacy Policy, which may be viewed at http://blackhawks.nhl.com/club/page.htm?id=46907.

(11) Sweepstakes is void where prohibited by law. 

(12) The Released Parties are not responsible for any typographical or other error in the printing of the offer, administration of the Sweepstakes, or announcement of the prizes. The invalidity or unenforceability of any provision of these Official Rules or the Affidavit will not affect the validity or enforceability of any other provision. In the event that any provision of the Official Rules or the Affidavit is determined to be invalid or otherwise unenforceable or illegal, the other provisions will remain in effect and will be construed in accordance with their terms as if the invalid or illegal provision were not contained therein. The Chicago Blackhawks' failure to enforce any term of these Official Rules will not constitute a waiver of that provision. Entrants agree to waive any rights to claim ambiguity of these Official Rules. In the event there is a discrepancy or inconsistency between disclosures or other statements contained in any Sweepstakes-related materials, privacy policy or terms of use on the Sweepstakes website and/or the terms and conditions of the Official Rules, the Official Rules shall prevail, govern and control and the discrepancy will be resolved in the Chicago Blackhawks' sole and absolute discretion. The Sweepstakes is governed by the laws of the state of Illinois.

(14) BY ENTERING THE SWEEPSTAKES, ENTRANT AGREES THAT TO THE EXTENT PERMITTED BY APPLICABLE LAW: (A) ANY AND ALL DISPUTES, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THE SWEEPSTAKES, OR ANY PRIZE AWARDED, WILL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION; (B) ANY AND ALL CLAIMS, JUDGMENTS AND AWARDS WILL BE LIMITED TO ACTUAL THIRD-PARTY, OUT-OF-POCKET COSTS INCURRED (IF ANY) NOT TO EXCEED FIFTY DOLLARS ($50.00), BUT IN NO EVENT WILL ATTORNEYS' FEES BE AWARDED OR RECOVERABLE; (C) UNDER NO CIRCUMSTANCES WILL ANY ENTRANT BE PERMITTED TO OBTAIN ANY AWARD FOR, AND ENTRANT HEREBY KNOWINGLY AND EXPRESSLY WAIVES ALL RIGHTS TO SEEK, PUNITIVE, INCIDENTAL, CONSEQUENTIAL OR SPECIAL DAMAGES, LOST PROFITS AND/OR ANY OTHER DAMAGES, OTHER THAN ACTUAL OUT OF POCKET EXPENSES NOT TO EXCEED FIFTY DOLLARS ($50.00), AND/OR ANY RIGHTS TO HAVE DAMAGES MULTIPLIED OR OTHERWISE INCREASED; (D) ENTRANTS' REMEDIES ARE LIMITED TO A CLAIM FOR MONEY DAMAGES (IF ANY) AND ENTRANT IRREVOCABLY WAIVES ANY RIGHT TO SEEK INJUNCTIVE OR EQUITABLE RELIEF; AND (E) ENTRANT IRREVOCABLY AGREES TO BRING ANY ACTION ARISING OUT OF OR RELATED TO THE SWEEPSTAKES IN THE CIRCUIT COURT OF COOK COUNTY, ILLINOIS, OR THE UNITED STATES DISTRICT COURT FOR THE NORTHERN DISTRICT OF ILLINOIS (IF JURISDICTIONAL PREREQUISITES ARE SATISFIED).
