St. Louis Blues Personalized License Plates

St. Louis Blues license plates are personalized plates available ONLY for registered Missouri vehicles with an annual contribution of $35. These plates can include up to six characters in length, which can include a space, hyphen (-), or apostrophe (').

Here is how to receive your Personalized Blues License Plate:

Fill out the Emblem Use Authorization Application and submit it along with your payment information. Contributions are $35 per year ($70 for two years*) and portions of your contribution benefit the Blues 14 Fund. * If your car is an even model year, you can apply for a two-year license in an even calendar year. If your car is an odd model year, you can apply for a two-year license in an odd calendar year. You can apply for a one-year license at any time.
Once your payment has been processed, the Blues will send you an Official St. Louis Blues Emblem Use Authorization Form (EUAF) along with an application for Missouri Personalized and Special License Plates (Form 1716).
Then take your application (Form 1716), EUAF, and $15 personalized plate fee to your local license bureau OR mail your application, EUAF along with your $15 to the Missouri Department of Revenue at: Motor Vehicle Bureau, P.O. Box 100, Jefferson City, MO 65105-0100
Once your application has been approved, your plates will be ordered by the Department of Revenue. You will be notified by mail when your plates are available for pick up at the local license bureau you indicated on your application. All questions about processing should be directed to the Department of Revenue via your local license bureau.

License Plates FAQs

In order to get personalized Blues license plates, you need to submit the following forms to the Missouri Department of Revenue:

St. Louis Blues Emblem Use Authorization Statement: Provides documentation that you have already paid the annual fee to the Blues.
Missouri Personalized and Special License Plates (Form 1716): Gives you the opportunity to specify what you would like on your personalized license plates.
Both forms are available by calling 314.589.5884.

Personalized plates can include up to (6) characters in length. If a license plate is requested which includes a space, hyphen (-), or apostrophe ('), the letters and/or numbers shall then be restricted to no more than five (5) characters, excluding the space, hyphen or apostrophe. No. The St. Louis Blues plates will simply replace your current plates. Registration stickers will come with your new plates. You will need to turn in your current plates upon receiving your new Blues plates.

No. Your personalized plate is yours to keep unless you fail to renew it each year. Your current personalized plate will simply be placed on a Blues plate providing it stays within the parameters of the six-digit limit.

The Emblem Use Authorization Form is valid for one year and can be used from the time it is purchased until the time when the plates expire. (Not to exceed one year from the date the letter was issued).

No. The Missouri Department of Revenue will send license renewal notices when plates are about to expire. Please allow enough time to receive your Emblem Use Authorization Form. The end of the month is busier than the beginning, so please plan ahead.
