In consideration for the undersigned Participant to have the opportunity to participate in an event ("Event") utilizing the ice-skating rink and other facilities at the TD Garden ("Garden Facilities"), Participant and Guardian agree as follows:

Participant and Guardian acknowledge there are certain risks in Participant's participation in the Event and use of the Facilities. These risks include the danger of being injured by ice skates, hockey sticks, hockey pucks or other athletic equipment, by the ice, boards or other Facilities, or by other participants in the Event.

Participant and Guardian hereby (i) assume all risks and danger or personal injuries and all other hazards arising from or related in any way to Participant's participation in the Event and use of the Facilities in connection therewith, whether occurring prior to, during or after the Event, (ii) agree to indemnify and hold harmless Delaware North Companies, Inc. -Boston, Boston Professional Hockey Association, Inc., Boston Bruins Charitable Foundation Inc., and their respective parent, subsidiary and affiliated companies, directors, officers, stockholders, employees, contractors, agents and representatives (collectively the "Entities") against any liability, loss,damage, expenses, claims, demands, causes of action, settlements or judgments or any kind or nature (including reasonable attorneys' fees) arising directly or indirectly from or related to Participant's participation in the Event and access and use of theFacilities in connection therewith, and (iii) release and discharge the Entities from any and all claims whatsoever in connection with Participant's participation in the Event and access and use of the Facilities in connection therewith, including claimsfor personal injury or death or property damage suffered by Participant.

PHOTO RELEASE

In consideration for my participation in an event identified below ("Event") sponsored or presented by Boston Professional Hockey Association, Inc. d/b/a the Boston Bruins ("Boston Bruins") , I, as Participant, hereby grant the Boston Bruins permission to use my image, likeness, actions, voice and/or statements in any live or recorded audio, video, or photographic display or other broadcast, transmission, exhibition or reproduction made of or at the Event for any purpose and in any medium without authorization or compensation. In connection with the foregoing, I hereby authorize the Boston Bruins to edit, alter, exhibit, publish or distribute such works, and make derivative works thereof. In addition, I waive the right to inspect or approve any work in which my image, likeness, actions, voice and/or statements appears. I understand and agree that such works will be the property of the Boston Bruins. I hereby hold harmless and release and forever discharge the Boston Bruins, and its parent, subsidiary and affiliated companies, directors, officers, stockholders, employees, contractors, agents and representatives, from all claims, demands, and causes of action, which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason or this authorization, including any claim for invasion of privacy, violation of right of publicity, defamation or appropriation, or any similar claim, in connection with any such use.

Participant will be responsible for equipment and assistance necessary for Participant to fully participate in the Event.

Guardian is the parent or legal Guardian of Participant, with full power and authority to execute this waiver and release and perform such obligations on Participant's behalf.

Participant and/or Guardian have read and fully understand and agree to the foregoing.
